• An optional reason for contacting P.A.V.E.
• Any additional information you voluntarily provide in the “Or, in your own words” field
• Your confirmation that you are 18 or older and agree to be contacted

Our other contact or notification forms may collect your name, email address, optional phone number and the message or request you choose to provide.

We may also collect limited technical or referral information needed to operate the website, understand how visitors reached us and reduce automated spam.

The Getting Started form is intended to begin a conversation. Please do not use it to submit diagnoses, medications, medical history, evaluations, IEP or 504 documents, school records or other sensitive records.

P.A.V.E. provides coaching, not medical care, mental health treatment, diagnosis, therapy or emergency services.

We use submitted information to:

• Respond to inquiries
• Help determine whether P.A.V.E. may be an appropriate fit
• Schedule or prepare for an initial conversation
• Operate and improve our website and services
• Prevent spam, misuse or security threats
• Meet applicable business and legal obligations

We do not sell personal information or use submitted form information for third-party advertising.

P.A.V.E. may use trusted service providers for website hosting, data storage, email delivery, scheduling and other operational needs. These providers may process limited information as necessary to perform those services.

If you choose to use an external scheduling link, information submitted through that service is also subject to that provider’s privacy practices.

Access to website submissions is limited to authorized people who need the information to respond or operate P.A.V.E.

We retain information only for as long as reasonably needed for the purposes described in this policy and for applicable business, security or legal requirements.

You may ask us to correct or delete information you submitted through the website. We will respond to reasonable requests, subject to any information we need to retain for legitimate business, security or legal purposes.

The website’s inquiry forms are intended for adults. A parent or legal guardian should make inquiries concerning a student under 18. Students who contact P.A.V.E. for themselves through these forms must be 18 or older.
